A formula built for populations, applied to individuals

BMI was devised in the 1830s by the Belgian statistician Adolphe Quetelet specifically to describe the distribution of body size across a large population for statistical and actuarial purposes — it was never designed as a diagnostic tool for evaluating any one individual's health. That original purpose explains most of its well-documented limitations when applied to a single person: a population-level statistical measure that usefully describes broad patterns across thousands of people can be a genuinely poor fit for many specific individuals, without either fact contradicting the other. Understanding this origin is what makes the formula's modern limitations make sense — they are not implementation flaws, they are consequences of using a tool for a job it was never built to do.

What BMI structurally cannot see

The formula uses exactly two inputs — height and weight — and nothing else, which means it has no way whatsoever to distinguish a kilogram of muscle from a kilogram of fat, or to know anything about where on the body fat is actually distributed. This is why a muscular athlete or a strength-trained individual routinely registers as "overweight" or even "obese" by BMI despite carrying very little body fat — dense muscle tissue weighs more than fat, and the formula cannot tell the difference. It is also why two people with an identical BMI, one carrying weight predominantly around the abdomen and one predominantly elsewhere, can have meaningfully different actual cardiometabolic health risk despite an identical number, since abdominal fat distribution specifically correlates with different health outcomes than the same total weight distributed elsewhere on the body.

Why the standard thresholds are not universal

The commonly cited BMI category cutoffs — 18.5, 25, 30 — were derived largely from studies of European populations, and subsequent research has found that health risk actually rises at different BMI thresholds in other populations. Several health authorities across Asia now recommend lower cutoffs specifically because cardiometabolic risk factors — elevated blood sugar, blood pressure — have been found to increase at a meaningfully lower BMI in those populations than the traditional thresholds would suggest, prompting adjusted category boundaries in national guidance for those countries. The categories also do not straightforwardly apply to children (who need age- and sex-specific growth charts entirely), pregnant people, or adults over roughly 65, whose body composition changes in ways the standard adult categories do not account for.

How is BMI calculated?

Weight in kilograms divided by height in metres squared: BMI = kg / m². For imperial units the conversion is done first. The formula dates from the 1830s and was designed to describe populations, not individuals — which is the source of most of its limitations.

What does BMI fail to measure?

It cannot distinguish muscle from fat, so athletes routinely register as "overweight" or "obese" while being nothing of the sort. It ignores where fat is carried, which matters more for health risk than the total. And it says nothing about fitness, blood pressure, blood sugar or cholesterol.

Do the categories apply to everyone?

No. The standard thresholds were derived largely from European populations. Health authorities in several Asian countries use lower cut-offs — overweight from around BMI 23 — because cardiometabolic risk rises at a lower BMI. The categories also do not apply to children, pregnant people, or adults over about 65 without adjustment.

Is a "healthy" BMI the same as being healthy?

No, in both directions. Someone in the healthy band can have poor cardiovascular fitness and bad metabolic markers; someone above it can have excellent ones. BMI is a cheap population screening number, not a diagnosis.

What should I do with the result?

Treat it as one weak signal among many. If you are concerned about your weight or health, waist measurement, blood pressure, resting heart rate and blood work are all more informative — and a clinician can interpret them together in a way a single number cannot.
